Qualified Intellectual Disability Professional
This position requires someone who has had a Drivers License for least 1 year (post permit) and a clean driving record th meets Sevita's insurability policy.
Bring your passion and experience to make a positive difference in the lives of others as a QIDP (Qualified Intellectual Disability Professional) with REM Community Services! The QIDP provides oversight and support of individual programming and needs in assigned ICF programs. The QIDP will work closely with facility and day program staff to ensure consistency and ensure service design and delivery, providing each individual with an active approprie trement program. The QIDP will also ensure th any discrepancies or conflicts between programmic, medical, dietary, and vocional aspects of the individual’s assessment and program are resolved and will ensure follow-up to recommendions for services, equipment, or programs.
WHAT YOU WILL BE DOING:
- Write and upde individual’s POC, programs, da sheets, corresponding informed consents and staff training sheets; prepares for and participes in Plan of Care/ IDT team meetings
- Conducts training on individual programming for facility and day program staff
- Completes ongoing required documention for each individual served, such as monthly documention and progress notes.
- Ensures th all records are accure, up-to-de, and in compliance with legal and corpore requirements.
- Ensures follow-up to recommendions for services, equipment, or programs; and ensures th adeque supports and assistive devices are present to promote independence.
- Attend and participe in facility and vocional program staff meetings
- Conduct observions of all shifts each facility and the day program; provides feedback and recommendions to supervisors and staff regarding program implemention / observions
- Ensure Comprehensive Function Assessment’s (CFA) are completed and reviewed annually; upde as individual’s needs change
- Maintains knowledge of clinical standards and ensures compliance in terms of trement interventions with all regulory, contractual, corpore, and legal requirements.
- Assists with all legal, licensing, regulory, and certificions activities as needed. Monitors plan of correction for ongoing compliance.
- Develops and maintains working relionships with all clients, families, funding sources and regulory agencies, as approprie, and monitors their sisfaction with services.
- Performs other reled duties and activities as required
WHAT WE NEED YOU TO BRING:
Educion and Experience
- Must be a QIDP (bachelor’s degree in a human service reled field and one year experience in direct service with people with developmental disabilities)
- Knowledge and understanding of ICF rules and regulions, preferred
Personal Skills
- Skilled in written and oral communicions
- An ability to build positive relionships all levels within the organizion; establish rapport quickly with individuals and staff alike
- Strong customer service oriention - established level of pience, tention to detail, and strong follow through
